What is Iron Studies?

Important

Reference ranges and preparation can vary between laboratories and test combinations. Use the instructions and reference interval supplied for your own test order.

Iron studies can include ferritin, serum iron, transferrin or TIBC, and transferrin saturation. Ferritin reflects stored iron but can also change with inflammation, so results are interpreted with the rest of the profile and clinical context.

Iron studies are a group of measurements used to assess iron availability and iron stores. Depending on the laboratory, the panel may include serum iron, total iron-binding capacity (TIBC) or transferrin, transferrin saturation and ferritin. Ferritin is especially useful as a marker of iron stores, but it can also rise with inflammation, liver disease and other conditions.

The results are therefore interpreted as a pattern. Low ferritin with low transferrin saturation may support iron deficiency, while a normal or high ferritin does not always exclude iron deficiency when inflammation is present. CBC indices and clinical history often add useful context.

Iron studies are often paired with a CBC because they answer complementary questions. The CBC can show anaemia and red-cell size, while ferritin and transferrin-related measurements help assess iron stores and availability. In straightforward iron deficiency, ferritin is often low, but inflammation can raise ferritin and obscure depleted stores. A clinician may therefore consider CRP, transferrin saturation or additional tests when the pattern is unclear. Finding iron deficiency should also lead to consideration of why iron is low rather than focusing only on replacement.

What it measures

Common markers included

FerritinProtein used as a marker of stored iron.
Serum ironAmount of iron circulating in the blood at collection.
TIBC / transferrinMeasures related to iron-binding capacity.
Transferrin saturationCalculated from iron and binding measurements where included.
Why it may be requested

Why might this test be requested?

Iron studies may be requested when anaemia is being investigated, when iron deficiency or iron overload is being considered, or to monitor iron replacement in selected situations. Ferritin can also be influenced by inflammation.

Understanding results

How are the results interpreted?

Iron studies are interpreted as a group. Ferritin reflects stored iron but is also an acute-phase protein and can rise with inflammation, infection or liver disease. Transferrin/TIBC and transferrin saturation help show how much iron is being carried in the blood. A low ferritin is often useful evidence of iron deficiency, while a normal or raised ferritin does not always exclude deficiency when inflammation is present.

The CBC adds important context because haemoglobin and red-cell indices can show whether an iron problem has affected red-cell production. Trends and the clinical reason for testing guide follow-up.

Common values and expected results

Use your own lab report first

These are common adult examples for orientation only. The reference interval printed on the patient's own laboratory report takes priority because methods, units, age, sex, pregnancy and clinical context can change the expected range.

Result context

What can affect the result?

Inflammation

Ferritin can rise as an acute-phase reactant, which can mask depleted iron stores.

Recent iron treatment

Oral iron, IV iron or transfusion can change the panel and should be considered.

Time of day and fasting

Serum iron can vary during the day; some laboratories request a morning or fasting sample.

Important context

What this test cannot tell you on its own

No single iron marker gives a complete picture. Serum iron varies during the day and with recent intake, ferritin can be altered by inflammation, and reference intervals vary by assay, age and sex. Iron studies should not be used to start long-term iron supplementation without considering the cause of any deficiency or overload pattern.

Specimen & collection

Collection notes

A venous blood sample is used. Preparation varies by laboratory; some ask for morning collection or fasting because serum iron changes during the day. Tell the clinician about iron tablets, IV iron and recent transfusion.

Common questions

Frequently asked questions

Ferritin reflects stored iron, while serum iron and transferrin-related measurements describe circulating iron and transport. The pattern is more useful than one number alone.

Yes. Ferritin can rise with inflammation, infection and liver conditions, so iron deficiency can sometimes coexist with a normal or high ferritin.

It estimates the percentage of iron-binding sites on transferrin that are occupied by iron and is calculated from serum iron and binding capacity.

Some laboratories request fasting or morning collection, while others do not. Follow the instructions for the specific panel.

No. CBC describes blood cells; iron studies assess iron transport and storage. They are often used together when anaemia is being investigated.

Yes for eligible bookings. Confirm whether the partner laboratory wants a fasting or morning sample.
